Necessary Forms

Due to the important nature of child care the state of Missouri requires a licensed facility like ours to maintain specific records and information.  You can get a head start in the application process by downloading and filling out the forms appropriate for your child.

All children enrolled in our day care program must have the following forms:

Enrollment Form.
Copy of the child's current immunization records.
Medical Authorization and General Permission forms.
Blood Lead Test form.  – More information.
Annual Agreement.
Termination Policy form.
Authorization for Emergency Care and Transportation.
Emergency Contact Sheet.
Handbook Signature Page.

Infants and Toddlers ages 6 weeks to 2 years old will also need these additional forms:

Infant and Toddler Feeding and Care Plan.
Medical Exam Report Form.  (This must be filled out and signed by a physician.)

Pre-school children ages 2 to 5 years will also need these additional forms:

Medical Exam Report Form.  (This must be filled out and signed by a physician.)

School children ages 5 to 8 years will also need these additional forms:

Health Statement Form.
Permission Form to Ride School Bus to Day Care.

Children who also participate in the Angels Among Us Preschool at the First Christian Church:

Permission to be walked to/from the Angels Among Us Preschool.
